If direct: check AST, ALT, alkaline phosphatase, and GGT. Calm enzymes mean Dubin-Johnson or Rotor.
Split the benign direct pair: Dubin-Johnson has mostly coproporphyrin I and black pigment; Rotor has both coproporphyrins and no pigment.
If enzymes are loud: transaminases point viral, drug, or ischemic. Alkaline phosphatase plus GGT points obstruction.

Answer: Dubin-Johnson syndrome from an MRP2 export defect.
Stem two: pregnancy or OCP use worsens the same direct-bilirubin pattern because estrogen can stress transport.
Do not overcall hepatitis or cholangitis: normal enzymes plus a benign course point away from inflammatory or obstructive disease.
Rotor lookalike: normal-color liver and elevated total urinary coproporphyrins. Dubin-Johnson is predominantly type I. Treatment for both is reassurance.
