Neonatal Jaundice and Hyperbilirubinemia

Almost every newborn turns a little yellow. The clock is what tells you whether it is harmless, or a brain-staining emergency. Read the timing, read the bilirubin fraction, and you have the whole disease.

A boy born 18 hours ago is noticeably yellow over his face and chest. Mom is blood type O positive; the baby is A positive. The total bilirubin is 10 mg/dL and almost entirely unconjugated, up from 4 mg/dL eight hours ago. Between checks he feeds, but he looks pale.
What is the single biggest concern here?

Why Every Newborn Tilts Yellow

Jaundice is just bilirubin staining the skin. Bilirubin is the leftover from recycled red blood cells. To understand newborn jaundice, follow the molecule.

The cause and effect chain

Old red blood cells get torn apart and their heme (the iron-carrying core) is broken down into unconjugated bilirubin (also called indirect bilirubin). This form is fat-soluble, so it cannot leave the body in urine, and it can sneak across the blood-brain barrier. That is the dangerous form.

The liver fixes that. An enzyme called UGT1A1 (uridine glucuronosyltransferase) bolts a sugar onto bilirubin and turns it into conjugated bilirubin (direct bilirubin). Now it is water-soluble, harmless to the brain, and can be dumped into bile and out the gut.

So the rule is simple: more bilirubin made, or less bilirubin cleared, equals yellow. Newborns are wired to make a lot and clear it slowly.

Newborns are born with a high hematocrit, and fetal red cells live a shorter life (about 70 to 90 days) than adult cells (120 days). More cells dying means more bilirubin produced every day.

The conjugating enzyme is only running at a fraction of adult capacity in the first days of life. The factory that converts the dangerous form to the safe form is understaffed, so bilirubin backs up.

The newborn gut is sterile and rich in beta-glucuronidase, an enzyme that un-conjugates bilirubin so it gets reabsorbed instead of excreted. Bilirubin keeps looping back into the blood instead of leaving in the stool.

Two Bilirubins, Two Stories

The very first fork in the road: is the high bilirubin unconjugated or conjugated? Fractionate it and the differential splits in half.

Unconjugated (indirect)

usually benign, watch the number
  • Overproduction: hemolysis (ABO, Rh, G6PD, spherocytosis)
  • Slow conjugation: physiologic, Gilbert, Crigler-Najjar
  • Gut recycling: breastfeeding and breast milk jaundice
  • The fat-soluble form. This is the one that can cause kernicterus

Conjugated (direct)

always pathologic, never wait
  • Direct greater than 1 mg/dL, or more than 20 percent of total
  • Biliary atresia (the can-not-miss diagnosis)
  • Neonatal hepatitis, infection, TPN cholestasis
  • Metabolic: galactosemia, alpha-1 antitrypsin deficiency
  • Look for pale stools and dark urine
Conjugated bilirubin is water-soluble and harmless to the brain, but its presence in a newborn is a red flag: it means bile is not draining. Conjugated hyperbilirubinemia is never physiologic.

When Did The Yellow Start?

The single most useful question in neonatal jaundice is the age of the baby when the jaundice appeared. Each window points at a different cause.

First 24 hours
Always pathologic · think hemolysis
Physiologic processes are too slow to make a baby visibly yellow this fast. Yellow before 24 hours means bilirubin is being produced at high speed. That is hemolysis until proven otherwise: ABO incompatibility (mom O, baby A or B), Rh incompatibility (Rh-negative sensitized mom, Rh-positive baby), G6PD deficiency, or hereditary spherocytosis. Workup: total and direct bilirubin, blood type, direct antiglobulin (Coombs) test, CBC, reticulocyte count, and a smear.
Day 2 to 4, term, well baby
Physiologic jaundice
Unconjugated, mild, peaks around day 3 to 5, and fades by the end of the first week. The baby feeds well, the exam is normal, and the bilirubin rises slowly (under 5 mg/dL per day and stays below the treatment line on the hour-specific nomogram). This is a diagnosis of exclusion. Management is monitoring.
Day 2 to 4, poorly fed baby
Breastfeeding (suboptimal intake) jaundice
Same window as physiologic, but the baby is not getting enough milk. Poor latch leads to few feeds, fewer stools, and dehydration. Less stool means more bilirubin recycled through the gut, and dehydration concentrates it. Clues: excess weight loss, dry mucous membranes, few wet diapers. Fix the feeding: 8 to 12 feeds per day plus lactation support. Do not stop breastfeeding.
After day 7, thriving baby
Breast milk jaundice
Appears later (peaks in the second week, can linger for weeks) in a well-fed, weight-gaining, perfectly happy infant. Substances in the milk increase enterohepatic recycling and slow conjugation. Unconjugated, benign, self-limited. Keep breastfeeding and reassure. (If the bilirubin were conjugated, this label would be wrong.)
Still yellow after 2 weeks
Prolonged jaundice · fractionate it
Jaundice past 2 weeks (term) demands you split the bilirubin. If it is conjugated, that is biliary atresia until proven otherwise: act fast. If it is unconjugated, think breast milk jaundice, hypothyroidism, or ongoing hemolysis.
The classic timing trap
Same day 2 to 4 window, two different answers. Breastfeeding jaundice is the hungry, dehydrated, weight-losing baby (fix the feeding). Breast milk jaundice is the thriving, chubby, later baby (just keep going). Memory hook: breastfeeding jaundice is about not enough feeding; breast milk jaundice is about the milk itself in a baby who is doing great.

Kernicterus, Phototherapy, and the Atresia Clock

Two emergencies hide inside neonatal jaundice: bilirubin staining the brain, and bile that cannot drain. Both have a treatment clock.

Acute bilirubin encephalopathy → kernicterus

When unconjugated bilirubin climbs high enough, it crosses the blood-brain barrier and deposits in the basal ganglia (especially the globus pallidus and subthalamic nucleus). That is acute bilirubin encephalopathy.

It moves in stages. Early: a lethargic, floppy, poorly feeding baby. Intermediate: irritability, hypertonia with retrocollis (arched neck) and opisthotonos (arched back), a high-pitched cry, and fever. Advanced: apnea, seizures, coma, and death.

If the staining becomes permanent, the chronic form is kernicterus: choreoathetoid cerebral palsy, sensorineural hearing loss, upward-gaze palsy, and dental enamel defects.

Phototherapy: how blue light empties the skin
Phototherapy does not need the liver. Blue light (around 460 to 490 nm) hits bilirubin sitting in the skin and changes its shape. Tap through the stages.

High unconjugated bilirubin sits in the skin, fat-soluble and stuck, because the immature liver cannot conjugate it fast enough.

Phototherapy

  • Excreted in bile and urine without needing UGT1A1
  • Plot the level on the hour-specific (Bhutani) nomogram
  • Useless for conjugated jaundice (causes bronze baby syndrome)

Exchange transfusion

the rescue when the brain is at stake
  • For very high levels approaching the neurotoxic range
  • For any signs of acute bilirubin encephalopathy
  • For failure of intensive phototherapy
  • Removes bilirubin and antibody-coated red cells at once
  • IVIG helps in isoimmune (Rh, ABO) disease to avoid exchange

The 2-Week-Old Who Is Still Yellow

Work this one yourself. Each step locks after you choose. This is the biliary atresia pathway, where every day matters.

A 2-week-old is still jaundiced, with pale clay-colored stools and dark urine. What is the first move?
Fractionate first. Pale stools and dark urine scream that bile is not reaching the gut. You must know whether the bilirubin is conjugated. Phototherapy and a breast milk label both assume unconjugated, and assuming here can cost the liver.
The fraction comes back conjugated: direct bilirubin is 4.5 mg/dL (well over 20 percent of total). What is the leading diagnosis?
Biliary atresia. Conjugated hyperbilirubinemia is never physiologic, and Gilbert raises the unconjugated fraction. Pale stools plus dark urine plus conjugated bilirubin in a 2-week-old is biliary atresia until imaging says otherwise.
How do you confirm it, and what is the time-sensitive treatment?
Image, confirm, operate. Ultrasound (look for an absent or small gallbladder and the triangular cord sign), then a HIDA scan that shows tracer taken up by liver but no excretion into the bowel, confirmed by intraoperative cholangiogram. The fix is a Kasai portoenterostomy, and outcomes are far better when it is done before about 8 weeks (60 days) of life. Waiting destroys the window.
