Which diagnosis is most likely?

Acute, H pylori, autoimmune, and Menetrier disease

The pattern is easier when the cause, region, acid output, and complication stay together.

Acute erosive gastropathy follows mucosal-defense failure from NSAIDs, alcohol, severe physiologic stress, bile reflux, ischemia, or critical illness and produces erosions or bleeding without the chronic atrophy pattern.

H pylori gastritis often begins in the antrum and may increase acid and duodenal-ulcer risk; with long-standing multifocal atrophy it can reduce acid and increase gastric-ulcer, MALT-lymphoma, and adenocarcinoma risk.

Autoimmune gastritis selectively injures body and fundus oxyntic mucosa, while Menetrier disease causes giant rugal folds from foveolar hyperplasia, protein loss, and reduced acid.

Switch among the four patterns.

NSAIDs, alcohol, stress, ischemia, or bile; superficial erosions and bleeding; defense failure.

Chronic active inflammation alters gastrin and acid patterns, weakens mucosal defense, and can produce peptic ulcer disease or lymphoid accumulation.

Over years, selected patients develop multifocal atrophy, intestinal metaplasia, dysplasia, and intestinal-type gastric adenocarcinoma; acquired lymphoid tissue can also evolve into MALT lymphoma.

Curling and Cushing ulcers are stress patterns, not interchangeable rhymes

Both occur in severe illness, but the proposed route to mucosal injury differs.

Severe burns and hypovolemia can reduce splanchnic perfusion and impair gastric mucosal defense, producing Curling-type stress ulcers.

Severe intracranial injury or raised intracranial pressure can increase vagal signaling and acid output, contributing to Cushing-type ulceration.

Modern stress-related mucosal disease is multifactorial, and critically ill patients are assessed by actual bleeding risk rather than by eponym alone.

Which stress ulcer is classically linked to severe burns?

Curling burns the blood supply; Cushing irritates the brain-vagus-acid axis.

The gastric region predicts the phenotype

Antrum controls gastrin; body and fundus produce acid, intrinsic factor, pepsinogen, and histamine.

Antral-predominant H pylori infection reduces local somatostatin signaling and can increase gastrin and acid delivery to duodenum.

Body-predominant or multifocal atrophic gastritis destroys acid-secreting mucosa and shifts the stomach toward hypochlorhydria, hypergastrinemia, bacterial overgrowth, and metaplasia.

The lesser curvature and distal stomach are common locations for intestinal-type ulcerated cancers, while diffuse cancer infiltrates broadly and can stiffen the entire wall into linitis plastica.

Intestinal and diffuse gastric adenocarcinoma are different diseases

One follows chronic mucosal injury; the other loses cellular adhesion and infiltrates.

Intestinal-type adenocarcinoma forms glands and is associated with H pylori, chronic atrophic gastritis, intestinal metaplasia, smoking, and high-salt or preserved-food exposures.

Diffuse-type adenocarcinoma contains poorly cohesive cells, often including mucin-filled signet-ring cells, and frequently involves loss of E-cadherin function through somatic or inherited CDH1 alterations.

Diffuse infiltration thickens the wall without requiring a discrete mass, while either type can spread to liver, peritoneum, distant nodes, ovaries, umbilicus, or pelvic cul-de-sac.

Gland-forming mass or ulcer arising through chronic gastritis, atrophy, metaplasia, and dysplasia.

Diffuse type

Poorly cohesive signet-ring cells infiltrate the wall and produce linitis plastica.

Virchow node

Left supraclavicular nodal metastasis through thoracic-duct drainage.

Krukenberg tumor

Typically bilateral ovarian metastases, often with mucin-rich signet-ring morphology.

Sister Mary Joseph nodule

Firm metastatic deposit at the umbilicus.

Blumer shelf

Palpable pelvic metastatic deposit in the rectouterine or rectovesical region.

Acid output separates hypergastrinemic states

High gastrin may be compensation for no acid or stimulation of too much acid.

Autoimmune oxyntic atrophy and Menetrier disease reduce acid output, although only autoimmune parietal-cell loss classically produces profound compensatory hypergastrinemia and intrinsic-factor deficiency.

Antral-predominant H pylori can increase gastrin and acid, whereas extensive H pylori atrophy can eventually lower acid; the stage and distribution matter more than the organism name alone.

Classify each state by relative gastric acid output.

A gastrin value without the gastric pH is half an answer.
